The division was formed for the 2000-01 when the old Midlands 4 West was split into two regional divisions called Midlands 4 West (North) and Midlands 4 West (South) and gained its current title for the 2009-10 season due to further restructuring to the Midlands League. Promoted teams tend to move up to Midlands 2 West (North) while demoted teams typically drop to Midlands 4 West (North).

Notes

  1. Mistakenly referred to as Midlands 4 East (South) on RFU website.
  2. Nuneaton Old Edwardians and Leek were also promoted. The reason for more teams being promoted than usual was due to Midlands league restructuring. Old Yardleians and Leek went up to Midlands 3 West (North) where they joined champions Kidderminster while Nuneaton Old Edwardians went up to Midlands 3 West (South)
  3. There was no relegation due to restructuring of the Midlands league with teams instead being transferred to the new look Midlands 4 West (North) or Midlands 4 West (South) for the following season.
  4. No relegation as the division would be increased from 10 to 12 teams for the following season.
  5. Last season as Midlands 4 West (North). Would be renamed Midlands 3 West (North) for the following season.
  6. One of Newcastle (Staffs) titles was won when division was unified as Midlands 4 West.
  7. Kidderminster's title was won when division was unified as Midlands 4 West.
