The 2004 CAF Confederation Cup was the first edition of the newly created CAF Confederation Cup. Hearts of Oak of Ghana beat fellow Ghanaians Asante Kotoko 8-7 on penalties in the final after the two legs ended 2-2.

- ^ The second leg was abandoned in the 50th minute with the score at 0-0 due to torrential rain. The replay was postponed twice due to torrential rain; the game was then rescheduled for August 15, but Orlando Pirates FC failed to appear. Orlando were disqualified and fined $5000, with Sable FC awarded a 3-0 win.
