History
Polkowice were founded in 1947.
In 2004, Polkowice played their only season in the Ekstraklasa, the top tier of Polish football. They drew 0–0 against Swit to finish 14th, which qualified them for a relegation playoff against Cracovia.[1] They lost the relegation playoff 8–0 on aggregate and were relegated after only one season in the top tier.[2]
